Youth Empowerment Project Inc - Building Positive Tomorrows for the People of Today
YEP Inc. has contracts with New York State Agencies to provide these billable services:
Traumatic Brain Injury (TBI) Waiver
YEP has a contract with the NYS Department of Health for Home and Community-Based Services Medicaid Waiver for individuals with Traumatic Brain Injury (HCBS/TBI). This program is one component of a comprehensive strategy developed by the New York State Department of Health to assure that New Yorkers with a traumatic brain injury could receive services within New York in the least restrictive setting, as a cost-effective community-based alternative to nursing facility care. Included in this program are:
· Service Coordination
· Independent life skills training
· Community Integration Counseling
· Structured Day Program
OPWDD Working with Developmentally Disabled Children and Adults
YEP Inc has a contract with OCFS to deliver Respite services within the home and community for people suffering from Developmental Disabilities. This program allows the parents and caregivers a break to do things that they can’t normally do when caring for a child or individual with a developmental disability.
YEP also provides a program called Community Habilitation that is a goal-based service provided within the home and community to strengthen the individual's needs and expand their independence.
These programs are waiver programs and are subject to individual approval with services provided through OPWDD. Individual staff hired to provide these services will undergo drug testing, background checks, registry checks, Medicaid fraud checks, and will have to be trained under the YEP Inc’s policies and procedures. All programs are subject to NYS guidelines.
Nursing Home Transition and Diversion (NHTD)
YEP Inc. provides services through the Nursing Home Transition and Diversion (NHTD) Medicaid Waiver which is a Home and Community-Based Services (HCBS) program, administered by the New York State Department of Health (DOH). This is a state contract. These programs use Medicaid funding to provide support and services to assist individuals with disabilities and seniors, toward successful inclusion in the community. These waiver participants may come from a nursing home facility or institution, or choose to participate in the waiver to prevent institutionalization.

YEP supports the citizens of our community by providing the following free programs:
YEP Food Pantry
YEP is a New York State Certified Food Pantry. YEP follows all NYS guidelines while supplying food for every person that comes to our door with a need. YEP provides each person residing in the household with 3 meals a day for 3 days. This averages out to approximately 30 to 40 families per week. YEP food pantry numbers have increased over 300 percent this year and we are providing approximately 2000 meals monthly. YEP funds the Food Pantry through a NYS DOH HPNAP grant, USDA food, YEP employee donations into the Family Help Fund and numerous community donations. When these grants and donations are not enough, YEP makes up the difference the best it can.
Christmas Give Away
For the last 12 years, YEP has supplied Christmas to thousands of families. The year 2009 was a record year, with YEP supplying Christmas gifts to 1500 individuals. YEP collects all year round to make sure the families of single moms and rural area families as well as elderly raising their grandchildren are taken care of at Christmas time. All the children and families receiving these gifts get at least 10 to 15 gifts to open on Christmas morning and so do the caretakers. Although YEP gives all year round, at Christmas, YEP makes sure everything is new. YEP tries to make sure that every person gets a blanket and new sheets for their bed (some may even get a bed). Many of the items given are warm clothing, PJs, underclothing, socks, hats, mittens and hygiene products.
Wrap Around Services
This program originated by helping behaviorally-challenged children and their parents become educated in their child’s diagnosis and their behavioral problems as well as familiarizing them with school policies and the laws governing our county. That program was funded by the Community Foundation. Although we worked diligently to obtain funding, with our statistics and numbers, we were unable to successfully sustain funding through the school districts and also NYS Education Department. However, we continue to provide a small portion of this population with the above mentioned services and referrals. YEP serves approximately 30 children a year on a pro-bono basis.
